I'm thinking a puzzle game like columns but I've not found anything online that says it was in the game or that columns had a remix of this. I read that it was in Earthworm Jim but that's definitely not the game I'm thinking of.

    Does anyone have any ideas?

I'm not sure, but it's a class piece of music. Mad to think it's 300 years old! It was definitely in Fantasia but I cant think what game.

From 3mins it sounds like Tomb Raider 2, possibly the Venice level...

But it's not, I've just checked. Oh well, the very very start bit is in Peggle :DD.

It's the Dracula theme so it's possible the music appears in one of the Castlevanias...

Yeah Castlevania most likely. You've probably heard it on the Sega Mega Drive Collection.

It always reminds me of Phantom of the Opera.
